DALLAS, TX — (Marketwire) — 03/04/13 — Phytel, the leader in automated, provider-led , announced today that three of its solutions — Outreach, Insight, and Coordinate — have achieved prevalidation status from the (NCQA) for autocredit toward 2011 patient-centered medical home (PCMH) criteria. All three solutions are part of Phytel-s platform, which enables providers to better manage quality and proactively improve chronic and preventive care across a population by aggregating data from clinical and financial systems, and automating critical population health management functions.
This latest prevalidation advances NCQA-s previous recognition of Phytel as a PCMH leader. Last January, NCQA made Phytel the first non-EHR vendor — and only the second company at the time — to have struck an arrangement allowing physician practices using Phytel-s solutions to automatically meet certain NCQA requirements for recognition as a PCMH. As a result, many of Phytel-s clients — who collectively see more than 30 million patients — have already pre-qualified for PCMH recognition credit from NCQA.
PCMH is a team-based health care delivery model that is becoming increasingly popular as a means of delivering comprehensive and continuous medical care to patients and obtaining better health outcomes. NCQA-s PCMH 2011 Prevalidation program is designed to help physician practices identify products that alleviate or make more efficient administrative aspects of meeting PCMH 2011 requirements.
NCQA evaluates advanced registries, population health management tools, EHR systems and other related technology solutions that provide functionality that meet standard requirements. Following a successful evaluation, a product can be prevalidated and awarded autocredit that can be transferred to practices utilizing the prevalidated product, eliminating the need to document for the associated factors within their PCMH 2011 survey.

The three Phytel products that meet the NCQA-s prevalidation requirements and received autocredit toward PCMH 2011 scoring are v.6.1.3, which automates patient identification and reminders to reduce gaps in care; v.1.4, which provides actionable performance reporting by practice and provider; and v.0.9.2, which automates care management workflows.
NCQA President Margaret E. O-Kane said, “By providing software that helps physician practices build the patient-focused, coordinated care infrastructure expected of NCQA patient-centered medical homes, Phytel is helping practices be part of modern health care-s most important transformations. Medical homes are the way of the future, and Phytel-s population health management solutions are helping practices get there.”
Richard Hodach, MD, Chief Medical Officer of Phytel, commented, “Phytel-s latest recognition from NCQA makes it even easier for physician practices to achieve PCMH recognition with our solutions and enable care teams to do more without increasing their workload. Phytel is committed to health care delivery reform that helps physicians deliver better quality at lower cost. Our scalable technology automates the routine tasks of population health management that are difficult for practices to undertake manually yet critical to the transformation we all want.”
