NaviNet Highlights Care Collaboration Through a Panel Discussion: Delivering Value to the Patient

This panel will examine the delivery of care through the Horizon Healthcare Innovations Patient-Centered Medical Home (PCMH) Program, featuring the perspectives of a health plan, physician, populations care coordinator, and care advocate. Participants will be engaged in a mindset-altering dialogue between the collaborative efforts between Horizon Healthcare Innovations and Vanguard Medical Group and the direct effect that this PCMH and its population care coordinator have on the outcomes of two patients and their patient advocate.
